Please bring your insurance cards with you when coming to the center. In most cases, we will be able to give you an estimate on the cost of your procedure beforehand. Our staff will verify your insurance and finalize arrangements before your procedure.
Billing for the facility, physician, anesthesiologist and laboratory (if applicable) is separate. We charge a set fee for each procedure performed at our center. Your physician, anesthesiologist and pathologist, when applicable, will bill you separately for their services.
The facility fee includes routine equipment, supplies, and drugs, operating and recovery room services. The fee does not include the professional services of your physician, anesthesiologist, and pathology.
As a courtesy to you, we will bill your insurance carrier for you; however, we cannot tell you what your policy will cover. Since your insurance policy and coverage is between you and your carrier, we urge you to contact your insurance carrier prior to your procedure for further consultation on any coverage, co-payment, deductible, and coinsurance amounts. You will be responsible for any co-pays and/or deductibles.
The Endo Surgi Center accepts cash, check, or credit card.
You will receive an easy to understand, all-inclusive bill for the services provided. We require all payment arrangements to be made prior to your procedure.
We accept most insurance plans, including Medicare. Primary and secondary insurance will be billed. Patients who do not have full insurance coverage or have no insurance should make financial arrangements with us. Co-payments are expected at the time of the visit. We offer the option to pay bills online. With our online bill pay option, you can securely make a payment for your service online with no hassles.

This list may not be comprehensive. For example, specialty insurers such as workers’ compensation and personal injury protection (“PIP”) are not included because there are a number of different types of insurance products and we may not participate in all of them. If you do not see your plan listed on our website, we strongly recommend that you contact us or your health insurance plan to confirm whether your plan is in-network or out-of-network.
In addition, the physicians who provide care within our center might not participate in the same insurance plans as the ASC. You should check with the physician who is arranging your healthcare services to see which insurance plans the physician participates in. You should also know that these healthcare professional(s)’ costs are not included in the facility’s charges. They will bill separately.
Other Care Providers
We work with other providers as part of your care. In addition to a bill from the facility and the physician, you may receive a bill from another provider, depending on the nature of your procedure.

The Cost of Your Procedure
Because you are responsible for costs for covered healthcare services until you reach your deductible, you should comparison shop if you elect to have a nonemergency procedure.
You can use tools like Healthcare Bluebook and the Medicare Procedure Price Lookup (for Medicare patients). Many health insurance websites also provide information on where to find in-network services.
